Skyler Della Fave is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) with a passion for integrated, patient-centered care. With over six years of experience in psychiatric and behavioral health nursing, as well as brain injury rehabilitation, Sky is dedicated to providing compassionate and evidence-based mental health treatment.
Sky earned a Bachelor of Science in Nursing (BSN) from the University of Connecticut and a Master of Science in Nursing (MSN) with a focus on psychiatric mental health from American Sentinel Post University. Throughout her career, Sky has worked in both inpatient psychiatric and rehabilitation settings, specializing in behavioral management, crisis intervention, and coordination of complex patient care.
With a strong commitment to holistic wellness, Sky specializes in hormone replacement therapy (HRT) for individuals also seeking inclusive hormone-related care and wellness management. Sky also offers expertise and experience in supporting brain injury rehabilitation. Her approach integrates psychiatric expertise with medical knowledge to provide comprehensive mental health treatment tailored to each patient’s unique needs. Sky is currently scheduling clients ages 14 and up for functional psychiatric medication management and 18 and up for HRT and functional psychiatric medication management. She is able to see clients located in the state of Delaware.
